Abstract

Introduction: The increasing feminization of medicine has demonstrated significant advances in women's participation in the profession. However, substantial gender inequalities persist in various aspects such as leadership, remuneration, and working conditions, compromising equity and the full professional development of female physicians. Objectives: To analyze the main gender disparities in contemporary medicine by identifying institutional, cultural, and social factors that perpetuate these inequalities, as well as to present effective strategies for their overcoming. Methods: An integrative review of recent studies published between 2019 and 2025, addressing female participation across different medical specialties, leadership in clinical research, salary disparities, and institutional policies promoting gender equity. Results: Despite the gradual increase in female representation, women remain underrepresented in leadership positions, particularly in industry-funded clinical trials, and face significant salary disparities. The overload resulting from balancing professional and family responsibilities contributes to dissatisfaction and hinders career advancement. Discussion: The identified inequalities reflect deep-rooted cultural and structural barriers within medical environments. Integrated strategies—including mentorship policies, salary transparency, harassment prevention, and flexible working hours—are essential to reverse this scenario. Conclusion: Overcoming gender inequalities in Medicine requires continuous and multidimensional affirmative, actions that foster inclusive and equitable work environments, ensuring justice, professional recognition, and enhancement of healthcare quality.
