Abstract

Low back pain is one of the most prevalent musculoskeletal conditions in the world population, considered a significant public health problem due to the functional, social, and economic impacts associated with pain and disability. In the context of Primary Health Care, therapeutic strategies based on physical exercise have been widely recommended for the management of low back pain, including the Pilates method. This study aimed to investigate, through a narrative literature review, the use of the Pilates method as a health promotion strategy for individuals with low back pain in Primary Health Care within the Brazilian Unified Health System (SUS). The study is characterized as theoretical and bibliographical research, conducted using searches in the PubMed/MEDLINE, SciELO, and PEDro databases, using descriptors related to Pilates, low back pain, and Primary Health Care. Articles published between 2020 and 2025 were included, as well as institutional documents and guidelines related to the topic. The studies analyzed demonstrated that the Pilates method presents positive results in reducing lower back pain, improving functionality, strengthening the stabilizing muscles of the trunk, increasing flexibility, and improving quality of life. In addition, the method's potential contribution to promoting self-care, encouraging physical activity, and preventing new episodes of lower back pain was observed. It is concluded that the Pilates method demonstrates potential as a complementary therapeutic strategy in the management of lower back pain, and can contribute to health promotion actions and comprehensive care in Primary Health Care.
