INFECÇÃO PELO VÍRUS DA DENGUE LEVANDO A COLECISTITE AGUDA ACALCULOSA

O Brasil é um dos países mais acometidos pela dengue, responsável por aproximadamente 23 milhões de casos no mundo entre 1980 e 2017. A infecção pelo vírus da dengue (DENV) pode cursar com cefaleia, febre, mialgia, artralgia e rash cutâneo, mas também pode apresentar-se de forma atípica, com manifestações gastrointestinais, como a colecistite aguda acalculosa (CAA). Essa condição é caracterizada por inflamação da vesícula biliar na ausência de cálculos, podendo ou não estar associada a infecção. As taxas de incidência da CAA variaram de 6% a 52%, diferença atribuída ao uso de distintos critérios diagnósticos. Diversas hipóteses foram propostas para explicar sua fisiopatologia, incluindo a possibilidade de invasão viral direta associada à estase biliar, endotoxemia e, em alguns casos, translocação bacteriana secundária. Além disso, observou-se correlação entre o espessamento da vesícula biliar, a gravidade da infecção e o risco de choque hipovolêmico, condição potencialmente fatal. Dessa forma, este artigo buscou enriquecer o conhecimento a respeito da CAA, a fim de prevenir complicações graves ou mesmo letais.
