The healthcare reception of transgender people in Brazil's Primary Health Care (PHC) still faces structural, symbolic, and institutional barriers, hindering equitable access and humanized care. Despite normative progress, challenges in implementing public policies persist. This is a narrative literature review conducted between February and March 2025, using the BVS, SciELO, and PubMed databases. Ten articles published between 2018 and 2024 that directly addressed the reception of trans people in Brazilian PHC were included. The main challenges identified were the lack of professional training, absence of specific care protocols, cisnormative practices, and symbolic violence. Conversely, strategies such as active listening, the use of social names, continuing education, and collaboration with community networks proved effective in promoting inclusive care. Overcoming inequities in the reception of trans people requires investment in professional training, development of clinical protocols, and full implementation of the National Policy for Comprehensive LGBT Health. Strengthening inclusive practices is essential to ensure equity and dignity in healthcare.
