NURSING THERAPEUTIC AND CLINICAL PRACTICE IN THE CARE OF VENOUS LEG ULCER IN AN OLDER ADULT

Long-standing venous lesions are characterized as chronic lesions due to the fact that they persist for months or years until they heal. The objective of this study was to analyze the therapy experienced and used based on the Systematization of Nursing Care (SAE) in a case study of venous lesions at the complex wound clinic in the city of Palmas, Paraná. This is a case study with data collected in October 2023. Patient care followed five steps: data collection/nursing history, diagnoses (NANDA, 2017-2018), care plan, implementation of care, and assessment of injuries using the Pressure Ulcer Scale for Healing, Bates-Jensen (BWAT), and pain scales. The importance of continuous assessment of lesions to define the appropriate therapy was evident. The application of PE enabled comprehensive care for the elderly, whose lesions caused pain, insomnia, and difficulty walking. With healing, there was an improvement in symptoms. The need to control NCDs during treatment was highlighted. A cleaning protocol, hydrogel, 10% papain, Unna boot, and 20% urea moisturizer were used on intact skin. Venous lesions are common in elderly people with CNCDs, especially hypertension and diabetes, aggravated by smoking and occupational factors. The PE guides therapy, promotes healing, controls NCDs, and improves quality of life. To define the appropriate therapy for the treatment of long-term injuries, it is necessary to use scientific knowledge, appropriate technologies, and the constant improvement of nursing professionals.
