UPFRONT CYTOREDUCTIVE NEPHRECTOMY IN CHROMOPHOBE RENAL CELL CARCINOMA WITH GASTRIC METASTASIS: CASE REPORT

Abstract

Introduction: Cytoreductive nephrectomy in metastatic renal cell carcinoma (RCC) may be indicated in selected patients, especially those with low prognostic risk, aiming at symptom control, reduction of tumor burden, and better integration with systemic therapy. Case report: A 75-year-old male patient presented with severe left flank pain for approximately 1 month, complaining of abdominal "bloating," without fever, vomiting, or significant weight loss. Abdominal computed tomography (CT) with contrast revealed a large hypervascular solid mass in the left kidney, in addition to a large lesion in the body and fundus of the stomach. The patient underwent a left total nephrectomy associated with partial gastrectomy with gastroplasty. The histopathological examination of the nephrectomy revealed chromophobe renal cell carcinoma, pT3a N0 M1. Conclusion: This case illustrates the rarity of gastric metastasis from chromophobe RCC and demonstrates the importance of CT scans in detecting atypical metastatic sites, prognostic evaluation (low risk according to the IMDC), and cytoreductive nephrectomy associated with metastasis resection in a selected patient.
