ENCAPSULATING PERITONEAL SCLEROSIS ASSOCIATED WITH THE PRESENCE OF FOREIGN BODIES: A SYSTEMATIC REVIEW

Abstract

Encapsulating peritoneal sclerosis (EPS) is a rare and potentially fatal condition characterized by the progressive formation of a fibrotic capsule surrounding the intestinal loops, leading to obstruction and severe functional impairment. This systematic review aimed to analyze the literature on EPS associated with foreign bodies, focusing on pathophysiology, diagnosis, and surgical management. A total of 41 studies published between 1907 and 2025 were included, encompassing case reports, clinical series, and observational reviews. Results demonstrated a strong correlation between synthetic materials such as non-absorbable sutures, drains, and catheter fragments and the development of chronic inflammatory reactions leading to progressive peritoneal fibrosis. Extensive enterolysis combined with complete removal of the foreign body proved to be the treatment of choice, significantly reducing mortality when performed early. In conclusion, EPS related to intraluminal foreign bodies represents a complex surgical entity that demands early diagnosis, comprehensive operative intervention, and multidisciplinary follow-up to optimize clinical outcomes and prevent recurrence.
