ESCLEROSE PERITONEAL ENCAPSULANTE ASSOCIADA À PRESENÇA DE CORPOS ESTRANHOS: UMA REVISÃO SISTEMÁTICA

Resumo

A esclerose peritoneal encapsulante (EPE) é uma condição rara e potencialmente fatal caracterizada pela formação progressiva de uma cápsula fibrosa envolvendo as alças intestinais, resultando em obstrução e comprometimento funcional grave. Este estudo tem como objetivo revisar sistematicamente a literatura sobre a EPE associada à presença de corpos estranhos, com ênfase na fisiopatologia, diagnóstico e abordagem cirúrgica. Foram incluídos 41 estudos entre 1907 e 2025, abrangendo relatos de caso, séries clínicas e revisões observacionais. Os resultados demonstraram forte correlação entre a presença de materiais sintéticos, como fios de sutura não absorvíveis, drenos e fragmentos de cateteres e o desenvolvimento de processo inflamatório crônico com fibrose peritoneal subsequente. A enterólise ampla, associada à remoção completa do corpo estranho, configurou o tratamento de escolha, com significativa redução da mortalidade quando realizada precocemente. Conclui-se que a EPE relacionada a corpos estranhos é uma entidade cirúrgica complexa que requer diagnóstico precoce, intervenção completa e acompanhamento multidisciplinar para otimizar os desfechos clínicos e prevenir recidivas.
