The experience of complex trauma represents a significant challenge to mental health, with the potential to trigger severe and long-lasting disorders. In many cases, these traumatic experiences can precipitate psychotic symptoms that profoundly compromise the functionality and quality of life of affected individuals. This report describes the clinical case of a female patient who, after being held captive for two years and subjected to different forms of violence—physical, sexual, and psychological—developed psychotic manifestations, including auditory, visual, and olfactory hallucinations, as well as persecutory delusions. Furthermore, a significant deterioration in her daily functioning and interpersonal relationships was observed. This condition highlighted the possible interconnection between extreme trauma, Complex Post-Traumatic Stress Disorder (C-PTSD), and psychotic disorders. The present report reinforces the importance of integrated therapeutic approaches and continuous follow-up by specialized multidisciplinary teams to facilitate the psychosocial and emotional rehabilitation of these patients.
