CORONECTOMY AS A BONE PRESERVATION STRATEGY IN ORAL REHABILITATION: A CLINICAL CASE REPORT

Abstract

Coronectomy, also called intentional partial odontectomy or intentional root burial, consists of removing the dental crown while preserving the roots in the alveolus, aiming at bone and periodontal preservation. This study reports a clinical case demonstrating the application of coronectomy as an auxiliary resource in oral rehabilitation (OR), highlighting its functional and biological advantages. A 64-year-old male patient attended the Dental Specialties Center of the Faculty of Dentistry of the Federal University of Pará for rehabilitative treatment due to the absence of teeth 36, 37, 43, 46, and 47. The therapeutic plan indicated the fabrication of a removable partial denture (RPD) as the first option; however, tooth 35, initially planned as an abutment, presented periodontal compromise and was not in a condition to receive a clasp. Therefore, it was decided to perform an intentional partial coronectomy on tooth 35, allowing the use of tooth 34 as an abutment for the future RPD. The procedure was uneventful during surgery, and no signs of infection, root exposure, or soft or hard tissue resorption were observed during the two-month postoperative follow-up. Coronectomy proved to be a predictable and financially accessible alternative for preserving supporting structures, maintaining proprioceptive stimuli, and ensuring functional stability in oral rehabilitation. It is concluded that, when correctly indicated, the technique represents a viable long-term strategy, contributing to prosthetic success and improving the patient's quality of life.
