Chronic low back pain is one of the most prevalent musculoskeletal conditions globally, associated with high rates of functional disability and significant socioeconomic impact. Therapeutic exercise is considered a first-line strategy in clinical management, with conventional strengthening and the Pilates method standing out as widely used approaches. This study is a narrative review of the literature, conducted in PubMed/MEDLINE, SciELO, the Virtual Health Library (VHL) – including LILACS and MEDLINE – and Google Scholar, covering publications between 2015 and 2025. The results demonstrated that both modalities are effective in reducing pain and improving function, with Pilates providing additional benefits such as increased flexibility, core stability, body awareness, and quality of life. Conventional strengthening remained a safe, accessible, and effective alternative. It is concluded that both interventions can be incorporated into the physical therapy treatment of chronic low back pain, and the choice should consider the patient's clinical profile and resource availability.
