Abstract

Patient V.R.A.S., a 19-year-old male with no underlying diseases, sought treatment at a private practice for endodontic treatment of a tooth with extensive periapical bone radiolucency. The dentist who initiated the treatment was unable to fill the canal due to drainage of a purulent collection via the root canal. A cone-beam computed tomography scan was requested to evaluate the initial appearance of the case. The probable diagnosis was a periapical cyst. One year after the procedure, another radiograph was requested, which showed only a reduction in the periapical lesion (Figure 4). It was then decided to perform enucleation of the periapical cyst through a periendodontic surgery: apicoectomy followed by retrograde obturation with MTA, aiming to achieve periapical bone repair. We conclude that periodontal surgery is an excellent alternative after attempting conservative non-surgical treatment, as it has a high success rate when performed correctly. Surgery using modern techniques and biocompatible materials proved effective in managing extensive apical reinfection, providing satisfactory resolution of the clinical picture.
