This research is a study on the strengthening of humanized reception practices in Primary Health Care (PHC). The research was applied by identifying the main demands of the Basic Health Unit (UBS), evaluating the perception of users and performing risk classification based on socioeconomic and health aspects. This research has the general objective of promoting accessible, empathetic and resolute care in PHC. According to the bibliographic study developed, it was possible to show that humanized reception and risk classification are fundamental strategies to transform health practice, especially in vulnerable territories. For the theoretical basis, a bibliographic review was used with articles published between 2017 and 2025, available on the CAPES Periodicals platform, with the keywords “health care”, “social vulnerability”, “risk stratification” and “health promotion”. The methods used were descriptive, observational study, by experience report. In the practical phase, an in-person investigation was carried out at the Dr. Francisco José C. Sousa Basic Health Unit in Teresina (PI), observing the reception practices, structure, planning and difficulties in accessing the service. Twenty-six patients participated in the action, the majority of whom were classified as “No Risk” (18) and 8 as “Minor Risk”. Data collection identified factors such as high blood pressure, diabetes mellitus, unemployment, poor sanitation and illiteracy. Finally, the research found that the humanized reception had a positive impact, with 90% of users feeling well received and 100% reporting respectful care, although there are still challenges regarding waiting times and resolution. Thus, the research resulted in strengthening the bond between the community and academics, highlighting the importance of interdisciplinary actions in PHC.
