The aim of this article was to report a clinical case of failure of primary treatment and endodontic retreatment performed in a single session. A 42-year-old patient was referred to the office of an endodontic specialist for retreatment of the mandibular molar. The radiographic examination demonstrated inadequate obturation. The filling material was removed with a Prodesgn rotary system and repreparation was performed with a Solla Collors rotary file 50/04 and foraminal patency was achieved with a Solla Collors Glidepath file 16/02 in the apical foramen. The filling was performed using the single cone technique associated with Bio-C Sealer cement. It was concluded that endodontic retreatment determined success, since clinical and radiographic follow-up confirmed repair of the extensive periapical lesion, as well as the furcation.
