This study aims to present a clinical case of retreatment with regression of an endodontic lesion, which simulated an endoperiomal lesion, with repair of the lesion after six months of completion of endodontic therapy. A 57-year-old female patient was referred to an endodontic specialist for treatment. The clinical examination revealed the presence of a fistula and the patient reported pain on vertical and horizontal percussion, the presence of a periodontal pocket of more than 10 millimeters on the distal face of tooth 47. The radiographic examination revealed extensive periapical bone radiolucency also involving the furcation region and severe atresia of the root canals. The treatment was performed with rotary instrumentation (Prodesign Logic ®. Associated with irrigation with 2% chlorhexidine gel. The root canal system was filled using the single cone technique associated with bioceramic cement. It is concluded that endodontic treatment alone determined clinical and radiographic success, ruling out the need for periodontal therapy.
