A male patient, 18 years old, with no history of systemic diseases, came to the private office for endodontic treatment of tooth 21. During the clinical examination, he reported trauma to this tooth more than 10 years ago. The periapical radiograph confirmed an open apex and the presence of a periapical lesion. After anesthesia and coronal access with a 1014 spherical bur, the root canal was explored with a number 80 K-file. Radiographic odontometry was performed with a 100 file and manual instrumentation was performed with a number 140 K-file associated with copious irrigation with 2% Chlorhexidine and saline solution in order to determine the sanitation of the root canals. The intracanal medication, BIO-C Temp, remained for a period of 15 days. The root canal system was obturated using the active lateral condensation technique, associated with BIO-C Sealer cement. It was concluded that treatment in immature teeth is viable, with clinical success with the combination of good intracanal medication and materials such as calcium hydroxide and BIO-C Temp, and a good adaptation of the main gutta-percha cone.
