BACTERIAL ENDOCARDITIS OF ORAL ORIGIN: CHALLENGES IN RISK STRATIFICATION AND CLINICAL DECISION-MAKING IN DENTISTRY

Oral bacterial endocarditis is a serious infectious condition associated with hematogenous dissemination of microorganisms from the oral cavity, mainly in patients with predisposing cardiovascular alterations. This study aimed to analyze the challenges related to risk stratification and clinical decision-making regarding antibiotic prophylaxis in contemporary dental practice. This is an integrative literature review, conducted in the SciELO, PubMed, Latindex, and OJS repositories, using articles published between 2022 and 2026, in Portuguese and English. The results showed an association between periodontal diseases, poor oral hygiene, and a higher risk of bacteremia related to the development of infective endocarditis. Furthermore, the studies demonstrated controversies regarding the effectiveness of antibiotic prophylaxis before invasive dental procedures, especially given the risk of bacterial resistance. It is concluded that individualized clinical assessment, associated with the promotion of oral health and multidisciplinary integration, represents a fundamental strategy for the prevention of bacterial endocarditis of oral origin and the improvement of dental care.
