OPHTHALMOLOGIC MANIFESTATIONS IN ANTIPHOSPHOLIPID ANTIBODY SYNDROME: A CASE REPORT

Antiphospholipid Antibody Syndrome (APS) is a systemic autoimmune coagulopathy associated with thromboembolic events and high gestational morbidity. Ocular involvement results from retinal and/or optic nerve vascular occlusions, which may progress to severe and irreversible visual impairment. In this context, the objective is to describe a case of bilateral neuroretinitis in a 26-year-old pregnant woman with APS who experienced a spontaneous abortion during her second pregnancy attempt. She presented to the emergency department with complaints of abdominal pain, gastrointestinal symptoms, vaginal bleeding, and sudden visual loss with peripheral visual field impairment in both eyes (OU). A comprehensive evaluation confirmed the diagnosis of APS, and ophthalmologic assessment identified bilateral neuroretinitis. Following treatment, there was improvement in visual acuity; however, significant sequelae remained at the level of the optic disc nerve fiber layer and the macula. Ocular manifestations in this autoimmune disease may indicate an underlying systemic coagulopathy, requiring early diagnosis and prompt multidisciplinary management to reduce the risk of blindness as well as fatal thromboembolic complications.
