CHALLENGES IN HOSPITAL-TO-HOME TRANSITIONAL CARE FOR ELDERLY PEOPLE WITH VENOUS INJURY: AN INTEGRATIVE REVIEW

Objective: To identify the challenges faced in the hospital-to-home care transition process for elderly people with venous ulcers, through a review of the scientific literature. Method: Integrative review with searches for articles in the LILACS, MEDLINE/BVS, PUBMED, and SciELO databases, in Portuguese and English, without publication year limits. The analysis of the 11 selected articles was performed using Content Analysis. Results: The results showed that, given the complexity of the hospital-to-home transition for elderly people with venous ulcers, resulting from challenges intrinsic to the individual and systemic failures, it is necessary to implement coordinated and person/family-centered strategies, individualized discharge planning, education for self-care, and continuous post-discharge support. A successful transition, driven by this integrated network, can prevent re-hospitalizations and reduce the mortality rate associated with ineffective care. Final considerations: Optimizing transitional care is essential to reduce readmissions and requires strengthening interprofessional communication, coordination between levels of care, and planning centered on the needs of the person and caregiver.
