Brazil faces a progressive decline in vaccination coverage rates, a phenomenon that threatens to reverse historical epidemiological achievements and favors the resurgence of vaccine-preventable diseases. This study is justified by the urgency of understanding the determinants of this decline and its consequences for public health, supporting the formulation of evidence-based public policies. The main objective is to analyze factors associated with the decline in vaccination coverage in Brazil and its implications for the resurgence of vaccine-preventable diseases. The methodology is based on exploratory bibliographic research, with qualitative analysis of scientific literature, technical reports from the Ministry of Health, epidemiological bulletins, and documents from international organizations. The results show progressive decline in coverage for different immunobiologicals, aggravated by the COVID-19 pandemic, regional heterogeneity with concentration of vulnerabilities in the North and Northeast regions, influence of anti-vaccine movements amplified by misinformation, and resurgence of diseases such as measles in states that maintained elimination. It is concluded that recovery of vaccination coverage requires a paradigmatic transformation in immunization strategies, combining strengthening of primary care, systematic combat against misinformation, social mobilization, and innovation in communication to ensure protection of vulnerable populations.
