Abstract

Introduction: Chronic kidney disease (CKD) is characterized by the progressive and irreversible loss of renal function, with hemodialysis (HD) being the main treatment used in advanced stages. Although essential for survival, the procedure is associated with physical inactivity, fatigue, and functional decline, which reinforces the importance of physiotherapy as a strategy to maintain functional capacity and quality of life. Objective: To analyze the main physiotherapeutic approaches applied to patients with CKD undergoing hemodialysis, identifying their effects on muscle strength, functional performance, and overall well-being. Materials and Methods: A systematic scoping review was conducted following the PRISMA 2020 guidelines. Randomized clinical trials published between 2015 and 2025 were included, retrieved from the PubMed database. The selected studies addressed intradialytic physiotherapy interventions such as resistance, aerobic, combined exercises, and complementary therapies. Results: Seventeen clinical trials were included, totaling 940 participants. Physiotherapy interventions showed significant improvements in strength tests (HGS), functional performance (6MWT, 30STS, TUGT), and quality of life (SF-36, KDQOL-SF, among others). Combined training approaches and neuromuscular electrical stimulation stood out for promoting more expressive and safer gains during HD. Conclusion: Intradialytic physiotherapy is a safe and effective intervention for the rehabilitation of patients with CKD, contributing to the preservation of physical function, reduction of fatigue, and improvement in quality of life. The development of standardized protocols and long-term studies is recommended to strengthen scientific evidence.
