Appendiceal mucocele accounts for 0.2–0.7% of appendiceal pathologies, making it a rare disease characterized by the accumulation of mucus in the vermiform appendix. It is a condition that is difficult to diagnose, as it is associated with nonspecific symptoms that mimic acute appendicitis. It is often an incidental finding during laparoscopies or, in asymptomatic patients, may be detected on routine radiological examinations. Objective: To report the rare case of appendiceal mucocele in a young patient, who underwent early diagnosis and had a favorable prognosis, thus enriching databases with information on clinical approaches and treatment protocols. Methodology: Description of a rare clinical case, with data collection from various scientific articles accessed through the PubMed and Scielo databases. Case description: 29-year-old man reported the presence of a palpable, painless, and mobile mass in the right iliac fossa for 20 days, associated with a 4 kg weight loss over 4 months, without changes in bowel habits or other associated symptoms. Abdominal ultrasound and total abdominal CT confirmed an appendiceal mucocele, and carcinoembryonic antigen was elevated (15.5 ng/mL). Conventional appendectomy was chosen as the treatment; the patient received oral diet on the first postoperative day and was discharged on the third postoperative day.
