Once implemented, nursing history needs to be assessed for its improvement. Objective: to construct an instrument to evaluate a nursing history model for hospitalized adult patients. Methods: descriptive methodological study completed in three stages: bibliographic research, choice of reference nursing history model and construction of the evaluation instrument based on the literature and experience of the researchers. Results: The evaluation instrument was finalized with 205 concrete indicators distributed in four parts: identification, general indicators, domains, classes and items evaluated in the patient, and the result of the evaluation. In each of these parts a five-point Likert type evaluation scale was inserted. Conclusion: The available product contains concrete indicators that can be arranged according to other reference nursing histories, however, before being put into practice, it needs to be submitted to a validation process.
