HIDRADENITIS SUPPURATIVA SYNDROME: THERAPEUTIC APPROACH AND MANAGEMENT INVOLVING DERMATOLOGY AND PLASTIC SURGERY

Introduction: Hidradenitis Suppurativa (HS) is a chronic, recurrent inflammatory disorder with a painful course and significant psychosocial impact. Its management requires a multidisciplinary approach combining dermatology and plastic surgery to achieve inflammatory control, adequate excision, and functional and aesthetic rehabilitation. Methods: A systematized literature review was conducted in PubMed, SciELO, Scopus, LILACS, and Web of Science databases, including full-text articles published between 2016 and 2025 involving human subjects. Fifteen studies addressing clinical, pharmacological, and surgical interventions in HS, focusing on interdisciplinary integration, were analyzed qualitatively. Results: Clinical treatment based on antibiotics and biologics such as adalimumab proved effective in controlling active inflammation but insufficient for addressing fistulous tracts and extensive scarring. Surgical management, particularly wide excision with graft or flap reconstruction, remains essential for definitive control. Combined protocols integrating biologic therapy and surgical preparation significantly reduced recurrence rates and improved quality of life. Innovative techniques—such as CO₂ laser, photodynamic therapy, and advanced reconstructive methods—enhance therapeutic outcomes and reinforce the importance of coordinated multidisciplinary care. Conclusion: The management of HS should be grounded in the collaboration between dermatology and plastic surgery, emphasizing inflammatory control, functional reconstruction, and long-term follow-up. This integrated and patient-centered approach redefines the therapeutic paradigm of HS, providing durable, aesthetic, and humanized results.
