THE ROLE OF PRIMARY CARE IN CARE COORDINATION AND ITS IMPACTS ON CONTINUITY OF CARE IN HEALTHCARE NETWORKS

Considering the persistence of fragmented care in Health Care Networks and its impacts on the quality of care, this study addresses the problem of insufficient articulation between care levels within the context of the Brazilian Unified Health System (SUS). The objective is to analyze the role of Primary Health Care in care coordination and its effects on continuity of care. To this end, a narrative literature review was conducted, using a qualitative, descriptive, and exploratory approach, based on searches in the PubMed/MEDLINE, SciELO, and Virtual Health Library databases, using DeCS/MeSH descriptors and defined inclusion and exclusion criteria, resulting in the selection of seven studies. Thus, it is observed that care coordination, when structured through longitudinal linkage, integration between services, and effective communication, contributes to greater coherence in care pathways, reduction of gaps in follow-up, and better use of health resources. On the other hand, structural, organizational, and informational limitations still compromise its effectiveness. This leads to the conclusion that strengthening Primary Care, coupled with improving care flows and integrating information systems, is a central element for consolidating continuity of care within healthcare networks.
