PALLIATIVE CARE FOR PATIENTS WITH DEMENTIA: MANAGEMENT STRATEGIES AND QUALITY OF LIFE

Introduction: The World Health Organization defines palliative care (PC) as an approach that improves the quality of life of patients and alleviates suffering in the face of life-threatening illnesses. In patients with dementia, a progressive and irreversible disease, early palliative care is essential, but there are still many obstacles to the accessibility and effectiveness of PC. Objective: To identify palliative care management strategies in people with dementia and assess patients' quality of life. Methodology: This is a narrative literature review based on database searches. Seventy-two articles were found, of which 18 were included, covering publications in Portuguese, English, and Spanish that addressed palliative care in patients with dementia and were published between January 2012 and June 2025. Results: The management of palliative care in dementia requires periodic reassessment of goals and directives of will, multiprofessional and longitudinal action, with a focus on training professionals and communication skills between the team, patient, and family, in order to promote interventions aimed at the physical, emotional, social, and spiritual needs of patients. The quality of life of patients with dementia is vulnerable to pain, psychological distress, malnutrition, sleep disorders, impaired breathing patterns and mobility, and dependence on caregivers. However, failure to identify dementia as a terminal syndrome leads to inappropriate interventions such as enteral feeding, non-palliative polypharmacy, and delayed application of PC. Thus, failures in the management of PC in patients with dementia were identified, such as lack of knowledge about palliative care; difficulty in establishing prognosis; communication failures; limited access; prejudices regarding death; and deficiencies in professional training. Conclusion: Palliative care in patients with dementia is essential for the relief of suffering and humanization of care through multidisciplinary and longitudinal management. In addition, it is important to promote greater access to and training in PC for dementia.
