TEMPORAL TREND AND INEQUALITIES IN IN-HOSPITAL MORTALITY DUE TO FEMUR FRACTURE IN BRAZIL

This study analyzed the temporal trend and demographic inequalities associated with in-hospital mortality due to femur fracture in Brazil’s Unified Health System (SUS) between 2008 and 2024. An ecological time-series study was conducted using data from the Hospital Information System (SIH/SUS). Annual mortality rates (%) and average annual percent variation (APV) were calculated, stratified by age, sex, and race/color, using weighted linear regression and interrupted time-series analysis to assess the COVID-19 pandemic’s impact. A total of 1,679,946 hospitalizations and approximately 51,000 deaths were recorded, with a mean lethality of 3.05%. A rising trend was observed until 2021 (APV = +1.8%; p < 0.05), followed by stabilization. Mortality increased sharply with age, reaching 7.22% among patients aged ≥80 years, and was higher in women (OR = 1.68) and white individuals (3.72%). In-hospital mortality due to femur fracture reflects the effects of aging, geriatric frailty, and structural health inequalities. Early surgical intervention, functional rehabilitation, and improved hospital information quality within the SUS are essential to reduce mortality.
