PERIPHERAL ULCERATIVE KERATITIS - MOOREN'S ULCER: CHALLENGES IN DIAGNOSIS AND TREATMENT

Peripheral ulcerative keratitis encompasses a group of predominantly autoimmune disorders characterized by progressive stromal destruction at the corneal periphery and a high risk of perforation. Among its variants, Mooren’s ulcer stands out as an exclusively ocular condition with an aggressive, unpredictable course and no established association with systemic disease. This review critically examined the epidemiology, pathophysiology, diagnostic challenges, and therapeutic strategies related to this entity, integrating both classical insights and contemporary evidence. Findings indicate that stromal degradation results from a complex autoimmune inflammatory cascade, supporting the early use of systemic immunosuppression tailored to disease severity. Surgical interventions remain essential in cases of impending perforation or therapeutic refractoriness. Despite significant advances, important gaps persist, especially regarding recurrence prediction and the establishment of standardized treatment protocols. Effective management requires multidisciplinary coordination, long-term monitoring, and continuous integration of emerging evidence to enhance visual outcomes.
