THERAPEUTIC MANAGEMENT OF INFECTIOUS ENDOCARDITIS: RECENT EVIDENCE

Infectious endocarditis (IE) remains a serious condition with a high in-hospital mortality rate, despite medical advances. This narrative review analyzes recent evidence on the therapeutic management of IE, highlighting changes in the epidemiological profile, with an increase in cases among the elderly and intravenous drug users, and the predominance of Staphylococcus aureus. The study discusses the critical importance of the multidisciplinary “endocarditis team” for improving clinical outcomes. A central focus is the paradigm shift brought about by the POET study, which validated the transition from intravenous to oral antibiotic therapy in stable patients, allowing early discharge from hospital. The specific challenges of IE caused by Enterococcus faecalis, which requires combination therapies or surgery to prevent recurrence, and the complexity of treatment in drug users, which requires the integration of addiction medicine into cardiovascular care, are also addressed.
