CONVERSION OF ROUX-EN-Y GASTRIC BYPASS INTO SLEEVE GASTRECTOMY DUE TO HYPOGLYCEMIA REFRACTORY TO CLINICAL TREATMENT: A CLINICAL CASE

Introduction: Obesity is a multifactorial chronic disease that is difficult to manage due to the high rate of refractoriness and adherence to treatment, which is associated with numerous metabolic and cardiovascular complications, resulting from the excessive accumulation of body fat resulting from an energy imbalance with repercussions on quality of life. Objective: To report a case in which a grade III obese, hypertensive and pre-diabetic patient underwent gastroplasty and subsequent reconstructive surgery due to refractory hypoglycemia. Case report: Male patient, 47 years old, underwent Bypass with Roux-en-Y reconstruction, progressing with satisfactory weight loss, but with refractory hypoglycemia, having undergone conversion to Sleeve with therapeutic success. Conclusion: Bariatric surgery is an important therapeutic tool in the treatment of obesity, and it is important to pay attention to its complications, such as refractory hypoglycemia, and how these can be managed, aiming for a positive outcome, highlighting the possibility of re-intervention to resolve them.
