Circulatory system diseases (CSDs) remain a leading cause of death worldwide, especially in low- and middle-income countries like Paraguay. This scenario is linked to population aging, the rise in risk factors such as hypertension and diabetes, and inequalities in healthcare access. Between 2016 and 2021, CSD deaths in Paraguay rose from 8,224 to 10,435, an increase of almost 27%. The vast majority of deaths occurred in people aged 60 or older (over 80%), followed by middle-aged adults. Projections for the coming years show that, by 2031, deaths could exceed 14,000, with an even greater impact among the elderly, but also with a progressive increase among younger adults. Despite the existence of prevention and health promotion programs, something is not progressing as it should: instead of decreasing, the number of deaths continues to rise, indicating shortcomings in the effectiveness of these actions or in the ability to adequately reach the populations most at risk. Thus, even with the relatively stable mortality rate, the absolute number of cases tends to rise, placing increasing pressure on health services. This reinforces the urgency of investing in early prevention, promoting healthy aging, and special attention to the most vulnerable groups. The study shows that addressing this challenge requires integrated public policies capable of reducing social inequalities and affecting different generations. Continuously monitoring indicators and adopting preventive measures can help reduce the number of CSD cases and improve the quality of life of the Paraguayan population.
