Cholecystectomy is a highly complex surgical procedure in dogs, primarily indicated to treat gallbladder mucocele (GBM), an emerging condition associated with endocrinopathies and dyslipidemias, which can culminate in biliary peritonitis. This study, a narrative literature review, synthesized the most recent evidence on surgical techniques and postoperative management. The results point to the transverse abdominal incision as a viable alternative to the midline incision, and the use of extradural anesthesia for superior analgesia. Elective surgical interventions present more favorable outcomes compared to emergency interventions, where biliary rupture is associated with a worse prognosis. Critical postoperative complications include hypernatremia (56% incidence), which requires rigorous hydroelectrolytic monitoring, and the presence of concomitant hepatopathies (fibrosis, copper accumulation), requiring additional therapeutic support. The prognosis is generally favorable for patients who survive the first 48–72 hours, provided that metabolic and endocrine comorbidities are controlled.
