Abstract

Introduction: Positive psychological factors may contribute to improved cardiovascular outcomes, potentially mediated by increased engagement in healthy behaviors. Gratitude interventions are among the positive psychology approaches with promising applications for individuals with cardiovascular diseases. Objective: To provide an updated overview of research on gratitude interventions in adult cardiovascular patients and to identify their main benefits. Methods: A scoping review was conducted following PRISMA-ScR guidelines. Searches were performed in the PubMed and PsycINFO databases using English-language terms, and included articles published between January 2015 and January 2025 that focused on gratitude interventions in adult patients with cardiovascular diseases. Only articles written in English, Spanish, or Portuguese were included. Results: This review included 11 studies on gratitude interventions involving patients with cardiovascular diseases, reporting both psychosocial and physiological outcomes. Findings suggest that gratitude can promote mental health by enhancing positive psychological states and helping individuals cope with daily stressors. It may also encourage healthy behaviors and improve biomarkers such as interleukin-6 and lipid profiles, as well as cardiovascular outcomes, including a reduction in heart attacks. Conclusions: Gratitude interventions can lead to improvements in inflammatory and lipid biomarkers, while also promoting mental health and the adoption of healthy behaviors.
