Healthy aging represents a global challenge that demands strategies to promote health and longevity in a sustainable way. This study aimed to analyze, through an integrative literature review, how the principles observed in the Blue Zones can be applied within Primary Health Care (PHC) to foster active and healthy aging. Twelve scientific articles published between 2015 and 2024 were selected, complemented by eight reference studies provided by experts in the field, addressing social, cultural, and behavioral determinants of longevity. The results indicated that natural diet, regular physical activity, spirituality, life purpose, and strong community bonds are central elements in promoting health and preventing chronic diseases. Integrating these principles into PHC may enhance the effectiveness of public health policies and strengthen person-centered elderly care, transforming health services into spaces of learning and shared responsibility. It is concluded that the principles of the Blue Zones provide valuable insights for rethinking the Brazilian health care model, prioritizing quality of life and well-being across all stages of aging.
