TRATAMENTO DO TREMOR ESSENCIAL: EVIDÊNCIAS RECENTES E DESAFIOS CLÍNICOS

O Tremor Essencial (TE) é o distúrbio de movimento mais comum, caracterizado por um tremor postural e cinético que pode causar incapacidade funcional significativa. Sua fisiopatologia está ligada a uma disfunção no circuito cerebelo-tálamo-cortical. Esta revisão narrativa, baseada em uma busca na base de dados PubMed, sintetiza as evidências atuais sobre o diagnóstico e as abordagens terapêuticas do TE. O diagnóstico permanece clínico, mas a neuroimagem avançada, como o DaTscan e a ressonância magnética funcional e estrutural, desempenha um papel crucial no diagnóstico diferencial e na compreensão dos mecanismos da doença. O tratamento é escalonado: a terapia farmacológica de primeira linha com propranolol e primidona oferece alívio sintomático para 50-70% dos pacientes, mas muitos descontinuam o tratamento por falta de eficácia ou efeitos adversos. Para casos refratários, intervenções cirúrgicas como a Estimulação Cerebral Profunda (DBS) e o Ultrassom Focalizado Guiado por Ressonância Magnética (MRgFUS) são altamente eficazes. Terapias não invasivas, como a estimulação nervosa periférica, representam uma fronteira emergente. O manejo do TE exige uma abordagem personalizada, considerando a gravidade dos sintomas e o perfil do paciente para selecionar a melhor estratégia terapêutica.
