The prescription of opioids in surgical dentistry has received increasing attention due to the need to ensure adequate postoperative pain control while reducing risks associated with the use of these medications, this study aimed to analyze aspects related to opioid pharmacology, risk assessment and strategies for preventing adverse events in the context of surgical dental practice. The research was conducted through a literature review, with analysis of recent scientific studies addressing prescribing patterns, clinical risk factors and patient safety measures, seeking to identify evidence that supports clinical decision-making. The results indicated that the choice of analgesic should consider procedural characteristics, patient systemic conditions and the possibility of using alternative therapies, demonstrating that limiting dose and duration of use, combined with patient guidance and clinical follow-up, represents an important strategy to reduce adverse events. It is concluded that safe opioid prescribing in surgical dentistry depends on the integration of clinical assessment, therapeutic planning, patient education and postoperative monitoring, promoting safer pain management and improved quality of care.
