Antisocial Personality Disorder (ASPD) is defined by a persistent disregard for the rights of others, manifesting itself through exploitative and delinquent behaviors, often devoid of remorse, as well as difficulty in following social norms and maintaining relationships. It is a condition associated with violence and recidivism, increasing the risk of interpersonal violence. ASPD goes beyond the idea of a simple behavioral deviation or a character flaw, and should be understood as a complex clinical condition, underpinned by profound alterations in neuropsychological functioning. This literature review aims to analyze the main challenges and strategies in the management of ASPD, based on data collected from the PubMed database using the MeSH descriptors "Antisocial Personality Disorder," "Treatment," and "Diagnosis." The findings indicate that the prognosis of ASPD is influenced by unfavorable early experiences, leading to "emotional shielding" in the face of authority figures. Thus, Antisocial Personality Disorder (ASPD) is configured as an intricate psychiatric condition, presenting social repercussions, challenging therapeutic interventions, causing significant functional impairments, and a strong association with violent behaviors. It is concluded that the prognosis tends to be unfavorable, especially due to demotivation and abandonment of treatment, reinforcing the need for continuous and multidimensional clinical strategies.
