Temporomandibular joint (TMJ) ankylosis is a debilitating condition characterized by fibrous or bony fusion between the mandibular condyle and the glenoid fossa, resulting in severe limitation of mouth opening, functional impairment, aesthetic alterations, and a negative impact on quality of life. Several surgical approaches have been described for the treatment of this pathology, including spacer arthroplasty, interposition arthroplasty with autogenous grafts, and total joint reconstruction with alloplastic prostheses; however, there is no absolute consensus regarding the ideal technique. This work aims to review and discuss the main surgical methods used in the treatment of TMJ ankylosis, based on recent literature, emphasizing functional outcomes, reankylosis rates, and technological advances applied to maxillofacial surgery. Aspects related to the choice of surgical technique, the use of interposition materials, alloplastic joint reconstruction, virtual surgical planning with three-dimensional guides, and the importance of postoperative functional rehabilitation are addressed.
