Placental accretism represents one of the most serious complications of pregnancy, characterized by the abnormal adherence of the placenta to the uterine wall and, in more advanced cases, invasion of adjacent structures such as the bladder and pelvic vessels. Its incidence has increased in parallel with the rising number of cesarean deliveries, with placenta previa being an important associated risk factor. Prenatal diagnosis, preferably by ultrasound and magnetic resonance imaging, is essential for proper delivery planning and for reducing maternal and fetal morbidity and mortality. This report describes a case of central total placenta increta diagnosed only intraoperatively, highlighting the importance of early recognition of this condition and multidisciplinary management to prevent severe hemorrhagic complications and unfavorable outcomes.
