ENDODONTIC RETREATMENT OF A LOWER MOLAR WITH A PERIAPICAL LESION AND THE PRESENCE OF A FUSED METAL CORE IN THE DISTAL ROOT

A 49-year-old patient reported pain during chewing and vertical percussion in the mandibular right second molar. Radiographically, the presence of a cast metal core, filling material below the appropriate apical limit, and diffuse periapical bone radiolucency were observed. The crown was removed with a transmetal bur and the metal core was removed using an operating microscope combined with two ultrasounds at combined power. The root canals were deobturated with a Logic RT 25.08, followed by electronic odontometry with a foraminal locator, and the patency of the root canals was assessed with a K-File #15 hand file. Repreparation was performed with a Sequence MK Life 35.04 rotary file. The irrigating agent used was 2.5% sodium hypochlorite. The root canal system was obturated using the single cone technique combined with Sealer Plus sealer. It is concluded that endodontic retreatment after removal of a cast metal core can be performed in a single session and determine the success of endodontic therapy.
