Melanoma is a malignant skin neoplasm with a high mortality rate, whose surgical approach fundamentally depends on the precise definition of the resection margin. The Breslow Thickness, which measures the depth of tumor invasion, is the main parameter used to guide the extent of the surgical margin and consequently the patient's prognosis. This narrative review aims to critically analyze the use of Breslow Thickness in determining the surgical margin in melanoma, discussing its clinical relevance, limitations, and therapeutic implications. Based on the analysis of recent literature, the importance of this index as an essential tool for surgical planning is highlighted, as well as the need for its integration with other clinical and molecular assessments to optimize treatment. It is concluded that Breslow Thickness remains the gold standard for margin definition, but melanoma management should be multidisciplinary and personalized.
