PRIMARY HEALTH CARE MANAGEMENT: CHALLENGES FOR COORDINATING CARE IN THE TERRITORY

Introduction: Primary Health Care (PHC) represents the foundation of the Unified Health System (SUS) and plays a fundamental role in coordinating care. However, it faces challenges that compromise its effectiveness, such as service fragmentation, communication gaps between levels of care, and management limitations in municipalities. Objective: To analyze the challenges faced in managing Primary Health Care for effective care coordination in the region. Methodology: This is an integrative literature review conducted through searches in SciELO, LILACS, PubMed, and Google Scholar. Articles published between 2017 and 2025, in Portuguese, English, and Spanish, were included. Results and Discussion: The studies analyzed show that the lack of clinical protocols, professional turnover, inefficient use of information systems, and disarticulation between services are factors that hinder care coordination. Successful experiences show that matrix support, the appreciation of the nursing team, and participatory territorial planning contribute to overcoming these obstacles. However, challenges remain related to political instability, low intersectoral integration, and limited consultation with workers and users. Conclusion: Strengthening PHC management requires investments in professional training, local governance, health technologies, and network organization. Overcoming the identified weaknesses is essential to consolidate PHC as a care provider and promoter of comprehensive and equitable health within the Unified Health System (SUS).
