Abstract

Liver transplantation is the treatment of choice for patients with end-stage liver disease, and is fundamental for reducing mortality and improving quality of life. This study aimed to analyze the landscape of liver donation in the state of Piauí, Brazil, from 2010 to 2025, focusing on epidemiological and clinical aspects, through an integrative literature review. The search was conducted in national and international databases, as well as official documents, including studies published between 2020 and 2025. The results showed a recent increase in the number of organ donations in the state, with particular emphasis on the increase in effective donors in recent years. The donor profile was characterized by a predominance of males, with a higher frequency of brain death due to stroke, followed by traumatic brain injury. A strong dependence on transplant centers in other states, especially Ceará, was also observed, highlighting regional inequalities in access to liver transplantation. Among the main limiting factors, family refusal and logistical challenges related to cold ischemia time stood out. It is concluded that, despite the observed advances, structural and operational challenges persist, requiring strategies aimed at expanding organ procurement, decentralizing services, and strengthening public policies to optimize liver transplantation.
