HIGIENE ORAL E ORIENTAÇÕES DE SUPORTE: PROTOCOLOS DE CUIDADO EM PACIENTES POLIFARMÁCIA XEROSTÔMICOS

A xerostomia induzida por polifarmácia configura-se como uma epidemia silenciosa no contexto do envelhecimento populacional global, com prevalência alarmante em instituições de longa permanência para idosos (ILPIs). Esta revisão narrativa sintetiza evidências científicas recentes (2021-2025) sobre protocolos de higiene oral e estratégias de suporte para pacientes polifármacos xerostômicos. A saliva desempenha funções críticas na homeostase bucal, e sua redução medicamentosa predispõe a cáries radiculares de progressão rápida, candidíase oral, desnutrição e pneumonia aspirativa. O presente estudo propõe um protocolo estruturado de intervenção multidisciplinar integrando cirurgiões-dentistas, médicos e farmacêuticos, com ênfase especial na educação permanente de cuidadores. Destaca-se a importância do "Mirror Sliding Test" como ferramenta de triagem de baixo custo, da revisão sistemática da farmacoterapia ("deprescribing") e da implementação de protocolos de higiene específicos incluindo flúor 5000 ppm e substitutos salivares. O modelo proposto visa romper o ciclo vicioso entre polifarmácia, xerostomia e complicações sistêmicas, contribuindo para a integralidade do cuidado ao idoso frágil.
